Academic Credit Information
To receive academic (course) credit for your internship,
follow the steps below:
Obtain Departmental Approval/Register for Course
Before registering for the Internship/Cooperative Education course:
- Download and complete the Course Registration Approval Form, then
- Obtain approval from your department.
NOTE: For all .pdf forms, we highly recommend using Internet Explorer, instead of Chrome.
If you use Chrome, you MUST first download the forms to your computer, then right click, select Open With, and select Adobe Acrobat (otherwise, your forms will not save and you will lose all your information).

After obtaining departmental approval, you must register for the Internship /Co-op course to ensure you get academic credit.
Your Faculty Advisor will give you the appropriate course code for registering.
Internship/ Co-op Assignments are below. Please print, complete, obtain any signatures, and return to Pioneer Center by the appropriate due date. Download the Academic Internship Complete Packet or select individual forms from the list below.
Assignments and forms will not be accepted or considered on-time without appropriate signatures.

- Fax to (940) 898-4110, Attention Kurt Krause, or
- Hand deliver to the Pioneer Center for Student Excellence (Blagg-Huey Library, 2nd floor)
Assignment #1: Wednesday, January 30, 2019
Assignment #2: Friday, May 3, 2019
Assignment #1: Monday, June 10, 2019
Assignment #2: Monday, August 5, 2019
Assignment #1: Wednesday, September 4, 2019
Assignment #2: Friday, December 6, 2019
Your Professor and/or Internship Advisor will administer grades upon completion of the Internship and successful completion of both assignments.
