Academic Credit Information
To receive academic (course) credit for your internship,
follow the steps below:
1. Download the complete Academic Internship Packet
2. Obtain Departmental Approval/Register for Course
Before you can register for the Internship / Cooperative Education course, you must obtain approval from your department (The Departmental Approval Form is included in the packet above.)
After obtaining departmental approval, you must register for the Internship /Co-op course to ensure you get academic credit. Your Faculty Advisor will give you the appropriate course code for registering.
Internship/ Co-op Assignments are below. Please print, complete, obtain any signatures, and return to Pioneer Center by the appropriate due date.
Assignments and forms will not be accepted or considered on-time without appropriate signatures.
- Scan and email to email@example.com, or
- Fax to (940) 898-4110, Attention Kurt Krause, or
- Hand deliver to the Pioneer Center for Student Excellence (Blagg-Huey Library, 2nd floor)

Employer's Final Evaluation (The Pioneer Center for Student Excellence will mail or e-mail the actual evaluation form to your employer.)
Your Professor and/or Internship Advisor will administer grades upon completion of the Internship and successful completion of both assignments.
