Below are 28 statements to describe the meaning that "health" or "being healthy" may have for an individual. Depending on your personal conception of health, you may agree or disagree with the statements. Beside each statement is a scale which ranges from strongly disagree to strongly agree. For each item, we would like you mark the answer which best represents the extent to which you disagree or agree with the statement. Please make sure that you answer every item. This is a measure of your personal conception of health; there are no right or wrong answers.
