Immunization Requirements & Verification Process for Health Science Majors
Texas law requires students involved in healthcare programs at Texas Woman's University to show proof of certain immunizations. These students are therefore required to submit written proof of their immunizations to Student Health Services for verification. Participation in clinic rotations is contingent upon compliance with this policy, and students are advised to submit their immunization records several weeks before their clinicals so that their immunization records can be updated and reported to their departments and professors.
Immunization records can be submitted by either fax, e-mail or mail to:
Student Health Services
PO Box 425467
Denton, TX 76204
Fax: 940.898.3849
A demographic cover sheet is required the first time that a student submits information to the Immunization Verification Program. The demographic page for each healthcare program can be found on our Forms page. Please make sure that the demographic page is signed at the bottom.
The demographic page is not required for additional submissions of records. However, please clearly label each page with the student's name and ID # or SSN.
Required Immunization:
- Measles - two doses
- Mumps - one dose
- Rubella - one dose
- Varicella (chicken pox)
- Tetanus/Diphtheria
- Hepatitis B (series of three injections)
- TB Screening - once per year
- Titers: MMR, Varicella & Hep B - after 28 days of completed series
Please view the Immunizations Overview for specific information on acceptable proof for each requirement.
