Application Information for Certificate Courses
Apply early as
all transcripts will need to be sent. Registration deadlines
for the semester apply. Information on deadlines
http://www.twu.edu/admissions/, choose
Class schedules
& catalogs, scroll down to Academic Calendars and choose the
appropriate semester.
All applications must be submitted online
via the Texas Common
Application.
Instructions for application:
1) Go to "Click here to enter apply
Texas"
2) Create a log in name and password. Remember it!
3) Scroll down and choose Graduate Application in gray
4) Choose the semester the course is offered
5) Major: Physical Therapy-Clinical
6) Page 3, #16 list the certificate course and instructor
7) Page 3, #17 Degree seeking : Certificate
8) Page 3, no test scores required.
9) Page 6 is optional but it is helpful to list your current
employer.
10) Page 7, there will be no questions
11) Page 8, there will be no essay in this step.
12) Page 9, payment options. The Graduate School fee is only
required once in a life time. If you are reapplying then
choose "Check" but don’t send the fee again.
13) You must choose "Ready to Submit" and Submit.
You must
send official transcripts for all colleges and
universities that you have attended. Have transcripts sent
directly to TWU Student Records or to the Denton School of PT.
