To receive academic (course) credit for your internship,
follow the steps below:

1. Obtain Departmental Approval/Register for Course
Before you can register for the Internship / Cooperative Education course, you must obtain approval from your department. Click here to download the departmental recommendation form.
After obtaining departmental approval, you must register for the Internship /Co-op course to ensure you get academic credit. Your Faculty Advisor will give you the appropriate course code for registering.
2. Academic Internship Application
Once you have applied and accepted a position, complete and submit the Internship/Cooperative Education Application online or in the Pioneer Center office (Library, Suite 200). Click here to download the application.
3. Student Training Agreement
Read and sign the Student Training Agreement and return to the Pioneer Center for Student Excellence. Click here to download the agreement.
Internship/ Co-op Assignments are below. Please print, complete, obtain any signatures, and return to Pioneer Center by the appropriate due date.
Assignments and forms will not be accepted or considered on-time without appropriate signatures.
1. Scan and email to firstname.lastname@example.org, or
2. Fax to (940) 898-4110, Attention Kurt Krause, or
3. Hand deliver to the Pioneer Center for Student Excellence (Blagg-Huey Library, 2nd floor)

Employer's Final Evaluation (The Pioneer Center for Student Excellence will mail or e-mail the actual evaluation form to your employer.)
Your Professor and/or Internship Advisor will administer grades upon completion of the Internship and successful completion of both assignments.
