Scope
This procedure covers the steps for completing payroll paperwork.
This procedure does not apply to Student Assistants and Contract Employees.
Types of Employees that Complete the Packet
- The following employees complete new hire paperwork in the Human Resources Department.
- Full-time Staff
- Part-time Staff (benefits eligible - 20 hours per week or more)
- Part-time Staff (not benefits eligible – 19 hours per week or less)
- Temporary Staff
- Full-time Faculty
- Part-time Faculty
- Adjunct Faculty
- Graduate Teaching Assistants (GTA)
- Graduate Research Assistants (GRA)
- Graduate Assistants (GA)
- Rehired employees must complete the packet
- The following employees do not complete the new hire packet in the Office of Human Resources.
- Student Assistants
- paperwork completed in Student Employment
- Contract
- paperwork processed through their own company
- Student Assistants
Area(s) Responsible for the Packet
- Human Resources Sr. Administrative Assistant
- Responsible for
- Maintaining the new hire packets to distribute to new employees
- Distributing the packet to new employees
- Verifying the packet is completed.
- Incomplete packets held at the front desk until completed
- All forms must be completed
- Will notify department/employee if packet is incomplete
- Payroll will also send email to Academic Affairs advising them that packet is not complete
- Incomplete packets held at the front desk until completed
- Human Resources Sr. Administrative Assistant enters the social security number in the pilot program
- Responsible for
- Human Resources Employment Department
- Include a new hire packet with the offer letter with instructions to submit the completed packet to the Human Resources Department by the third day of employment.
- Human Resources Payroll/Records Department
- Responsible for entering the paperwork into Oracle.

- TWU Biographical Data Form.
- FORM I-9
- Employee only has three days to complete.
- Election to Disclose Information Form
- Direct Deposit Form
- Direct Deposit Declination Statement
- State of Texas New Hire Reporting Form
- Form W-4
- Oath of Office Form
- Prior Texas State Service Agreement
- English Language Proficiency of Faculty Form
Process for Processing Packet
- Distribution of packet to employee
- A new hire packet is included with the offer letter with instructions to submit the completed packet to the Human Resources Department by the third day of employment, or
- New employees picks up a packet from the Human Resources Department
- Completion of forms
- TWU Biographical Data Form.
- FORM I-9
- The Human Resources Department must complete Section 2 by examining evidence of identify and employment eligibility within three (3) business days of the date employment begins.
- Election to Disclose Information Form
- Direct Deposit Form
- Direct Deposit Declination Statement
- State of Texas New Hire Reporting Form
- Form W-4
- Oath of Office Form
- Prior Texas State Service Agreement
- English Language Proficiency of Faculty Form
- Only new employees that will be teaching classes will complete the form.
- Submitting packet for processing
- Verify packet is complete
- Incomplete packet
- Human Resources Sr. Administrative Assistant holds packet at the Human Resources front desk until all forms are complete
- Completed packet
- Human Resources Sr. Administrative Assistant enters the social security number in the pilot program
- Verifies number and name matches
- Forward completed packet to Payroll
- Incomplete packet
- Enter data from packet into Oracle.
- Verify packet is complete
