Complete the Dependent Information section on page 2.

 If applying for an initial dependent I- 20, you also need:

• Marriage certificate for spouse and/or birth certificates for children.
• Please submit current Bank Statements and Affidavit of Sponsor.
• You must prove $4,000 for spouse and $3,000 per dependent.
