Current personal bank statement or bank letter on personal account from self or sponsor, not older than 3 months, with bank name and name of self/sponsor indicating personal bank balance in U.S. dollars.  TWU will NOT accept bank statements from a business account or financial documents that are insurance policies, retirement accounts or personal or real property appraisals.  Affidavit of Sponsor/Financial Certification Form.

Application for reinstatement of status

Next semester or term of intended enrollment:

(I understand that if I leave and reenter the country, I am required to enroll full-time during my first semester or term upon returning to TWU as listed above. I also realize that I cannot reenter the country more than 30 days prior to my new start date.)

 If applying for an initial dependent I- 20, you also need:

• Marriage certificate for spouse and/or birth certificates for children.
