HOW ARE REFERRALS MADE?

 An adult, a child's parent, a legal guardian or a referring professional may call the Intake Coordinator from 9 a.m. to 5 p.m., Monday through Friday. Referrals are taken from school personnel, physicians, rehab agencies, parents, and medical or educational facilities. Applicants may also self-refer. Inquiries may be addressed to: The Institute for Clinical Services and Applied Research, P. O. Box 425737, Denton, TX 76204-5737 (or call 940/898-2285). The Institute secretary's office is located in the MCL Building, Room 909, on the Denton campus.
