Forms provided by public safety:
- Card Access Request (Card Swipe) Word PDF
- A request for access to a room or building that requires card access can be made using the Card Access Request form.
- Keypad Alarm Access Request Word PDF
- A request for a user code for the alarm panels can be made using the Keypad Alarm Access Request form.
For either form, the Word version can be filled in electronically and printed, but the PDF version can only be printed and filled out by hand. Either version that is used needs to be signed and faxed to DPS at (940) 898 – 2999.
