* These forms are required to reserve campus facilities and Hubbard Hall.
** This form is required for events where alcohol will be served, money will be exchanged and/or attendance will be 100 or more.

Reservations for campus facilities are accepted on a first-come-first-serve basis. Each request must be submitted on the appropriate reservation form at least thirty days in advance.
