Geriatric Education: Transition of Care (online program)
The target audience for this program is any health care provider who interacts with geriatric patients and is concerned with improving patient outcomes and seeking ways to improve cooperation between hospitals and community-based resources. Healthcare professionals who can benefit from this program include physicians, such as Geriatric Fellows, nurses, social workers, and allied health providers, such as occupational therapists, physical therapists, dietetic aids, and dietitians.
