Workshop Objectives
- To provide an overview of transitional care, why it’s important, and opportunities for expansion and payment proposed in national health care reform.
- Describe effective transitions of care models for improving patient safety, satisfaction and health outcomes.
- To provide useful and practical information about successful approaches to transitioning patients from one care setting to another.
- Identify barriers to successful care transitions.
- Examine the roles and relationships between health care providers and community-based practitioners/resources in transitions of care.

General Information
Continuing Education Credit
All attendees will receive 4 clock hours (.4 CEU’s) from Texas Woman’s University. The continuing education credits are accepted by the Texas State Board of Examiners of Professional Counselors, Texas State Board for Social Worker Examiners, and Texas State Board of Examiners of Psychologists.
Location / Times
The workshop will be held at Texas Woman’s University, Houston Center located at 6700 Fannin Street, Houston, TX 77030. Registration will begin at 7:30 a.m. and the lecture at 8 a.m. in room 4330 with breakfast items provided in room 4314. Lunch will be served in the Legacy Room located on the 8th floor.
Registration Fee
Each course fee is $55.00, includes parking and meals, after February 19 the fee will increase to $65.

Cancellation/Refund Policy
Registrants who find it necessary to cancel will receive a refund, less a $20 processing fee. To receive a refund, you must submit written notification seven days prior to the event start date (email: lifelonginfo@twu.edu or fax: 940.898.3416). Substitutions may be made at any time. Refunds will not be issued once the event begins. The Office of Lifelong Learning reserves the right to cancel this event if minimum enrollment is not met or other events out of our control take place. In doing so, Texas Woman’s University’s liability is limited to the registration fee.

The Texas Consortium Geriatric Education Center (TCGEC) is one of 49 federally funded Geriatric Education Centers whose overall purpose is to improve the quality of patient care for older Americans through better prepared health care providers. Thus, we focus on health professions educators who are training the next generation of practitioners, and on current community practitioners to increase their knowledge about geriatrics. Texas Woman’s University is one of nine schools in Texas that participate as consortium partners in this effort. Since 1985, the TCGEC has offered training in geriatrics to over 40,000 health care providers in Texas.
