Academic Problem Resolution Form
When a student wishes to appeal a problem with academic units that is not related to grades, the student uses the Academic Problem Resolution form (found on the Academic Affairs website under “Forms.” The appeal must be made within one week of the occurrence of the problem, and it must be put in writing following the instructions on the form for the time limit and the steps involved in making the appeal. Students are not required to continue the appeal through all the steps; the appeal may be discontinued at any stage.

Accreditation Reports Review Policy
Any program accreditation-related report, other than routine data submission, must be reviewed prior to submission by the appropriate dean, the graduate dean if appropriate, and the Provost. Reports should be sent to these individuals well in advance of their due dates to allow for adequate review and revision as needed.
